During a recent inspection of the conveyance listed above, an inspector for the Department of Labor & Industries found conditions that must be corrected to ensure safety and compliance with applicable codes. The enclosed inspection report will tell you what must be corrected. The building owner is responsible, under RCW 70.87.120, for ensuring that all corrections are made. If you are unsure which corrections must be made by a licensed elevator mechanic please contact your elevator service company for assistance. Please note, you should provide your elevator service company with a copy of this inspection report for their information as we do not provide them with a copy.

To protect the safety of those who use your building, and to avoid substantial penalties required by law (RCW 70.87.185 and WAC 296-96-01070), and to prevent us from removing your elevator from service (RCW 70.87.125) YOU MUST RETURN THIS COMPLETED REPORT BY THE DUE DATE.

2.14a The data plate shall indicate the Code in effect at the time of installation. The data plate shall also indicate the Code in effect at the time of any alteration and indicate the applicable requirements of 8.7. The data plate shall be in plain view and securely attached to the main line disconnect or on the controller. The data plate material and construction must have letters and figures stamped, etched, cast, or otherwise applied to the face and must remain permanently and readily legible. The height of the letters and figures shall be not less than 0.125 inches. See: A17.1d–2000 and earlier editions — 215; A17.1–2000 and later editions — 8.9 and 8.7.1.8; WAC 296-96-00650 (code in effect). Inspectors note discrepancies to the code data plate and indicate the installation date or any alteration performed after 7/1/2004:
Install code year data plate and /or alteration data plate on exterior of unit.
8.6.1b 8.6.1.2.1 A written Maintenance Control Program shall be in place to maintain the equipment in compliance with the requirements of 8.6. (1) Examinations, maintenance of equipment at scheduled intervals in order to ensure that the installation conforms to the requirements of 8.6. Owner; Examinations and maintenance are not in scheduled intervals.
Perform annual cleandown and testing and provide documentation on site.
