Apple Health Sign Language Interpreting Service Access


To request Sign Language Interpreter service that is covered through the Health Care Authority's Interpreter Service Program you must use this request form. Apple Health/Medicaid requests require the patient's DOB and ProviderOne number.

If two or more siblings are being scheduled with back-to-back appointments, you must submit a separate form for each sibling with separated ProviderOne numbers.

Multiple requests can be used when there is one patient with multiple appointments at the same location. If you have feedback or concerns about our venders, fill out this

Eastern (In-Person Interpreting) - Adams, Ferry, Lincoln, Pend Oreille, Spokane, Stevens, and Whitman counties.

Olympic (In-Person Interpreting) - Clallam, Grays Harbor, Jefferson, Kitsap, Mason, Pierce, and Thurston counties.

North Central (In-Person Interpreting) - Chelan, Douglas, Grant, and Okanogan counties.

Northwest (In-Person Interpreting) - King, San Juan, Skagit, Snohomish, and Whatcom counties.

Southwest (In-Person Interpreting) - Clark, Cowlitz, Lewis, Klickitat, Pacific, and Skamania counties.

South Central (In-Person Interpreting) - Asotin, Benton, Columbia, Garfield, Franklin, Kittitas, Walla Walla, and Yakima counties.

Virtual Meeting (Video Interpreting)
It is a state-wide service. You, the client (patient/consumer/employee/client), and the interpreter are all in different locations. Everyone joins the same video conference platform (such as a Teams or Zoom call), allowing for remote collaboration
