HCA Voucher Portal Contact Information Survey

This survey is for home care aide (HCA) training programs, employers, or others who want to pay the exam fees for their caregivers. The survey asks for contact information necessary for a HCA training program, employer, or third-party payers to have access to Prometric’s online Client Voucher Portal. The information enables the Prometric testing agency to email the appropriate person with directions on portal access, profile setup, and instructions for purchasing exam payment vouchers.
