Dealer Order Form

Pursuant to RCW 19.118.031, a new motor vehicle dealer is required to provide a Notice of Consumer Rights to a consumer at the time of purchase or lease of a 'new motor vehicle'. Failure to provide the Notice of Rights is a per se violation of Chapter 19.86 RCW, the Unfair Business Practices Act.

Notices of Consumer Rights forms (LL-219) are provided by the Attorney General's Office, Lemon Law Administration. You can order a supply of Notice of Rights forms by completing and submitting this form or by calling 1-800-541-8898. You may also provide our approved electronic Notice of Consumer Rights version in lieu of our printed forms, the buyer must still sign the electronic form.
