Currently, this Electronic Entry Permit system is for issuing permits for shipment of cattle and swine

There are 6 steps to obtain an Entry Permit with this web application:

Step 1 - Preliminary information questions and information
Step 2 - Enter the Origin Addresses of the animals
Step 3 - Enter the Destination Addresses of the animals
Step 4 - Enter animal information (number, species, type, sex) and number of loads
Step 5 - Enter a CVI / Brand Certificate / Invoice number and optional veterinarian information
Step 6 - Review information and then print the Entry Permit for your records
