Six Months of Coverage

Income verification is needed at original application, change in income, and 6 month review. Examples of income verification are: pay stubs from last 30 days, a letter from employer showing income, and self-employment work sheet. Examples of income changes are: new job, pay increase, or reduction in hours.

Your children are covered by SCHIP for up to 6 months from the month you applied. This coverage may end during the 6 months if your children move out of state, turn 19 years old, your income goes up, or monthly premiums are not paid.

Before your children's SCHIP coverage ends, you will receive an eligibility review form from DSHS. You will need to complete the review form and return it to DSHS to see if your children are eligible for another 6 months of SCHIP.

If at any time your family income goes down or a child under 19 becomes pregnant or disabled, your children may be eligible for Medicaid. Medicaid offers the same services as SCHIP with no premiums. Please call 1-800-562-3022 if you have a change.
