Health Plan ID Card 

If you enroll your children in a health plan, you will also get an Identification Card from the plan. You need both cards for medical appointments and when you pick up prescriptions. Your children may not get health care or their prescriptions if you do not have these cards with you. If you need care before you get your plan card, contact the Medical Assistance helpline or call at 1-800-562-3022 (TTY/TDD only 1-800-848-5429. They will tell you what to do to get care.
