How to apply for SCHIP
If your monthly
family income is close to the amounts on the
Qualifying Income standards chart,
your kids may qualify for free or low-cost health insurance. Even if your
income is above these
amounts, we still encourage you to apply.

Kids are considered for free health insurance first (Medicaid and Non-citizen programs)

Kids who don't qualify for free insurance are screened for low-cost health insurance
(SCHIP) (premiums)
Applying is easy!
You can
apply on line and submit your application directly to DSHS. Or,
you
can print a copy of the application, complete and mail it to the address below.
If you need assistance,
call 1-877-543-7669. When your completed application is received, it
will be screened for eligibility.
